Who this is for: Clinicians seeing a patient who already has an active care plan.
Before you start: The patient needs a confirmed, active plan from an earlier visit.
Why you'd use this
The value of a care plan is in visit six, not visit one. Knowing the patient is at phase 2 of 4, that shoulder flexion has moved from 40 to 55 degrees against a target of 80, and that a reassessment is due today — before you walk in — is the difference between continuing a course of treatment and restarting the conversation each time.
What you see before the visit
Open the appointment before you start recording and a Pre-visit recap appears. Its heading summarizes where the patient is, for example Phase 2 · Visit 6 · Reassess: this visit, and beneath it are four sections:
Reassessment — with Complete the reassessment or Book the reassessment for next visit
Goals & progress — each goal with Primary, Done, or Due: {date}
Objective measures — each showing Latest {value} against Target {target}, with a trend
Patient to-dos — what was prescribed at the last visit
What happens after the visit
Finish the visit. Eluve reads the new note and updates the existing plan rather than creating a new one.
On the Care Plan tab you'll see the plan you already have, with a banner over it: Updating this plan with findings from this visit… The plan stays readable while this runs.
When the update lands, review this visit's items the same way you did the first time, then select Confirm Care Plan.
Things to watch out for
The plan is read-only while it updates.
Between the visit ending and the update landing you can read the plan but not change it. The banner says so. Wait for it to clear rather than reloading.
New items arrive undecided.
Everything this visit captured shows as Added this visit and needs a verdict, exactly like the first visit. Items already on the plan show In plan and carry forward untouched.
Confirming again is what keeps the plan current.
If you never confirm, this visit's findings don't join the plan, and the next Pre-visit recap still reflects the older state.
The reassessment is a marker on the schedule, not a reminder.
The plan shows Reassess at visit {visits} based on the cadence you set. Nothing chases you — the Pre-visit recap surfaces it on the day as Reassess: this visit.
Where to find plans outside an appointment
The patient's Care Plans tab on their Patient Profile shows their plans independently of any one visit. Note the difference: Care Plan (singular) is the review tab on an appointment; Care Plans (plural) is the patient-level tab.
