Review and confirm a care plan

Decide what Eluve got right, fix what it didn't, and activate the plan so it carries forward to future visits.

Who this is for: Clinicians at practices with Care Plans enabled.

Before you start: The visit needs a generated draft on the Care Plan tab.

Why you'd use this

Eluve drafts the care plan, but the clinician has to confirm it to add it to the patient’s chart. Confirming is the step where you take ownership: the items you confirm become the active plan, and the next visit opens with the existing plan.

Steps

  1. Open the Care Plan tab on the appointment.

  2. Work through Review findings — everything Eluve captured from this session. Each row takes one of three verdicts:

    • Add to plan — it belongs on the care plan.

    • Patient record only — record it on the chart, but not as part of the plan. Shows as a Record only pill.

    • Reject — it doesn't belong

  3. Fix anything worded wrong with Edit. Saving an edit also confirms that finding to the care plan.

  4. Fill in the plan itself under Build the care plan: add a condition, a goal, a to-do, or an objective measure, and set the phase and schedule.

  5. Check the Care plan checklist at the top. It tracks six things: Set session frequency, Select phase, Add objective measures, Prescribe exercises, Add at least one goal, and Review all {N} findings.

  6. Select Confirm Care Plan. The plan activates and the checklist area shows Care Plan active.

Things to watch out for

⚠️ Findings you never decide on are dropped. Confirming the plan links only the items you actually confirmed. Anything still showing Needs review is not added to the plan, not recorded, and not rejected — it simply doesn't carry forward, and you get no warning. Before you confirm, you can filter by “Needs review” to show what’s left to review.

The checklist doesn't stop you.

Confirm Care Plan stays available whether or not the checklist is complete. It's a readiness prompt, not a gate — an incomplete checklist activates a thinner plan, which is a legitimate choice but should be a deliberate one.

A measurement captured as a range can't be tracked on a goal.

It shows a Range badge and the explanation Captured as a range. Enter a single value to track this on a goal. Enter a single number if you want the goal to track it.
