A care plan is the through-line between a patient's visits. Eluve drafts one from the visit note, you review and confirm it, and from then on it travels with the patient — showing up before the next appointment, updating itself from each new note, and tracking whether the goals you set are actually being met.
🧪 Care Plans is a beta feature. It is rolling out to practices gradually. If you don't see a Care Plan tab on your appointments, your practice doesn't have it enabled yet — contact support to request access.
Who can use it: Clinicians at practices where Care Plans has been enabled. There is no setting you can switch on yourself.
What it does
Care Plans works in three stages:
1. Eluve drafts the plan from the visit note. When a visit is completed, Eluve reads the note, pulls out the clinical findings, and drafts a plan.
2. You review and confirm it. The Care Plan tab shows everything Eluve captured this visit. You decide, item by item, what belongs on the plan, what belongs on the patient's record only, and what should be rejected outright. Then you click “Confirm Care Plan”, which activates the plan and carries it forward.
3. It follows the patient. At the next appointment, a Pre-visit recap appears before you start recording, showing the phase, the visit number, goals and their progress, objective measures, and patient to-dos. When that visit's note lands, the plan updates itself with the new findings and you review the changes the same way.
Why it helps
Without a care plan, every visit starts from a blank page and the only record of what you decided last time is prose buried in an old note. Progress against a goal — range of motion at visit 1 versus visit 6 — is something you have to reconstruct by reading back. A care plan keeps the goals, the measures, the prescribed exercises, and the reassessment schedule in one structured place that updates itself from your notes, so the next visit opens with the plan already on screen.
Two places you'll see care plans
These are easy to confuse, so they're worth separating:

Limits and known behavior
A care plan is generated from the visit note, not the recording. Nothing care-plan-related starts until the note exists.
Only a completed visit can generate a care plan. If the visit isn't finished, the action is unavailable.
One care plan per visit. If a visit already has one, the action reads Update care plan rather than Generate care plan, and attempting to generate again shows This visit already has a care plan.
One active plan per patient. A patient carries a single active longitudinal plan. Later visits update that plan rather than starting new ones.
Objective measures captured as a range can't be tracked on a goal. A reading recorded as a range shows a Range badge and this explanation: Captured as a range. Enter a single value to track this on a goal.
Frequently asked questions
I don't see a Care Plan tab on my appointment.
The tab only appears once there is something to show: a plan being prepared for this visit, or an active plan the patient already carries. On a first visit it appears once the note lands and generation begins. If you never see it on any appointment, your practice doesn't have Care Plans enabled — contact support.
Do I have to use a care plan for every visit?
No. The action sits in the tab bar rather than interrupting you, and a visit with no care plan behaves exactly as it always has.
Does confirming the care plan sign the note?
No. Confirming a care plan and signing a note are separate actions.
Can two clinicians work on the same patient's plan?
Yes — the plan belongs to the patient, not to you. Whoever sees the patient next opens the same active plan.
