Generate a care plan from a visit

Turn a completed visit into a draft care plan that Eluve builds from the visit note. When you're done you'll have a draft on the Care Plan tab, ready to review.

Who this is for: Clinicians at practices with Care Plans enabled.

Before you start: The visit must be completed. Eluve builds the plan from the visit note, so the note needs to generate first.

🧪 Care Plans is a beta feature. It is rolling out to practices gradually. If you don't see a Care Plan tab on your appointments, your practice doesn't have it enabled yet — contact support to request access.

Why you'd use this

Most of what belongs in a care plan — the conditions, goals, exercises and measurements — you already said out loud during the visit. Generating from the note means you adjust a draft instead of building one from an empty form.

Steps

  1. Finish the visit as you normally would. A care plan can only come from a completed visit.

  2. Open the Care Plan tab on the appointment.

  3. Wait for the note. You'll see Waiting for the visit note…, then Your care plan is generated from the visit note. On a long visit this can take a few minutes.

  4. When the draft lands, the tab shows Review findings and Build the care plan, ready to review.

To trigger it yourself, select Generate care plan in the tab bar. On a patient who already has a plan, the control reads Update care plan.

Things to watch out for

Nothing happens until the note exists.

The plan is built from the note, not from the audio. Until the note generates, no care plan can be generated. A visit with no note can't produce a care plan at all.

Generating the note again on the same visit replaces the draft of the care plan.

If this visit already produced a draft care plan, regenerating the note will create a completely new draft of the car eplan. Anything you'd already edit on that care plan is lost.

āš ļø Replacing a draft care plan cannot be undone. If you've already edited items on the current draft, confirm the plan instead of replacing it.

An active plan can't be replaced. If someone confirms it while you're replacing the draft, you'll see This care plan was activated before it could be replaced.

If it's not working

"Waiting for the visit note…" and it never moves on.

The note hasn't generated. Check the Note tab — if the note failed, fix that first; no care plan can be built without it.

"This visit has no generated note to create a care plan from" or "This visit has no current note to create a care plan from".

There's no note on this visit yet. Generate the note first, then return to the Care Plan tab.

"This visit already has a care plan".

This visit has produced one. Open the Care Plan tab to review it, or use Update care plan.

"Care plan review is taking longer than expected".

The message continues: You can continue the visit and try again shortly. Nothing is lost — come back to the tab in a few minutes.

"Care plan review isn't available for this organization yet".

Your practice doesn't have Care Plans enabled. Contact support.
