The Patient Profile is the redesigned patient page in the Eluve web app. It replaces the old stacked patient page with a persistent patient rail on the left — identity, demographics and contact details that stay on screen wherever you are — and a set of tabs on the right for the patient's visits, clinical record, care plans and documents.
🧪 Beta feature. The new Patient Profile is rolling out gradually. If your patient pages still show the old layout — a large name header above the tabs, with Imported Notes, Medical Profile and Artifacts in the tab strip — it isn't enabled for your account yet. Contact support to request access.
Where to find it
In the web app, open Patients from the left sidebar.
Search for the patient and open their row.
You'll land on the patient's Appointments tab.
You can also reach a profile from an appointment, from the schedule, and from a care gaps worklist — every patient link in the app opens the same profile.
Use All patients at the top left to get back to the list.
What's on the page
The profile has two halves that work together: the rail on the left never changes as you move around, and the tabbed area on the right is where you go deep.
The patient rail
The rail is the patient's face sheet. It stays put while you switch tabs, so you never lose sight of who you're looking at. It has three collapsible sections:
Patient information — status, date of birth with age, MRN, primary care provider, last visit and next visit.
Identity and demographics — legal sex, gender identity, pronouns, race, ethnicity, preferred language and marital status.
Contact and relationships — phone numbers, email, address and emergency contacts.
Sections only appear when there's something to show, so a sparse patient has a short rail. Fields that exist in the record but that you don't have permission to see are marked as withheld rather than hidden.
The tabs

Filters in the Patient Record
Each Patient Record section can be filtered, with a live count next to each filter:
Everything — every item of that type on the record.
In a Care Plan — items that are linked to one of the patient's care plans.
Patient record only — items that aren't attached to a care plan.
Provisional — items Eluve extracted from a note that are still waiting on clinician review. This filter only appears when there's at least one provisional item.
Where an item belongs to a care plan, you can open that plan directly from the item — and from the Patient Record you can add an unlinked condition, goal or activity to an active care plan.
Editing patient details
Click Edit profile at the top of the rail to update the patient's identity, demographics and contact details in one dialog.
Save is only available once you've actually changed something and the required identity fields are filled in.
Closing the dialog with unsaved edits asks you to confirm before discarding them. Cancel is the way out — there's no X.
If someone else edited the same profile while you had the dialog open, Eluve won't overwrite their changes. You'll be asked to close and reopen the dialog to edit the latest version.
Clinical information — conditions, goals, measurements — is not edited here. It comes from your notes and is reviewed in the Patient Record and Care Plans tabs.
Marking a patient deceased or as having left the practice
The arrow next to Edit profile opens a short menu:
Mark as deceased / Remove deceased status
Mark as left the practice / Mark as active
Both ask you to confirm, both show on the patient's status in the rail, and both can be undone later. Patients marked deceased or inactive are hidden from the patient list unless you tick Include inactive/deceased.

Old links still work. A bookmark or a link from an older note that points at a retired tab lands you on the closest equivalent in the new profile rather than a dead page.
💡Tip: if the rail looks thin, it usually means the patient's demographics were never filled in — not that Eluve is hiding them. Edit profile is the fastest way to complete a record before a first visit.
Frequently asked questions
Why does my patient page still look the old way?
The new profile is in beta and enabled account by account. If you see the old layout, it hasn't been switched on for you yet. Contact support to request access.
Where did the Imported Notes tab go?
Into Documents, together with generated documents. Old imported-notes links redirect there automatically.
Where did Medical Profile go?
Its clinical content moved into the Patient Record tab. The old standalone tab has been retired from the new profile.
Can I edit a patient's conditions or goals from the profile?
Not from Edit profile — that dialog covers identity, demographics and contact details. Clinical items come from your notes and are reviewed in Patient Record and Care Plans.
What does the Provisional filter mean?
Those are conditions and goals Eluve pulled out of a note that no clinician has reviewed yet. The filter only shows up when the patient has at least one.
I don't see a Care Plans tab.
Care Plans is a separate feature and has to be enabled for your organization. If it's off, the tab is hidden and the rest of the profile works as normal.
Who can see a patient's profile?
Access is unchanged from before. Organization admins can see every patient in the organization; other members see only the patients assigned to them, unless your organization has broader patient access enabled.
Does Privacy Mode still work here?
Yes. Privacy Mode masks identifying details such as name, date of birth and contact information while leaving clinical content readable.
Someone else edited the patient while I had the dialog open — did I overwrite them?
No. Eluve detects the conflict and refuses the save, asking you to reopen the dialog on the latest version.
A patient I marked as deceased disappeared from my list.
That's expected. Tick Include inactive/deceased on the patients list to see them, and you can undo the status from the profile at any time.
