Two points need to be mentioned:
An audio recording will happen in the background
That recording will be used to generate their note and improve the service

It records our conversation just for the purpose of creating your medical note, so I can spend less time typing and more time with you. Your information is private and protected, and only used for your care. Are you comfortable with that?
Feel free to use any of these or create your own that will work best for your patients.

If you’re just dictating your own thoughts, you do not need to request patient consent.
